Course description

This module introduces practical, team-based communication strategies that strengthen coordination, efficiency, and patient outcomes in integrated care settings. Participants will learn how to effectively use daily huddles and the SBAR (Situation, Background, Assessment, Recommendation) framework to support real-time decision-making, streamline workflows, and enhance interprofessional collaboration. This course emphasizes how structured communication improves care for complex patients, reduces fragmentation, and supports whole-person care delivery

• Understand the purpose and structure of team huddles and how they improve access, coordination, and proactive care planning
• Apply the SBAR framework to communicate patient information clearly, concisely, and effectively across team members
